INVITATION
TO REGISTER
In compliance to Executive Order No. 40, The Department of Health
(DOH) invites interested manufacturers, suppliers, distributors,
and contractors with the DOH for possible participation in future
eligibility screening and bidding for goods, consulting services,
and civil works.
The following documents, as minimum requirements, for registration
shall be submitted to Director Agnette Peralta, Chairperson,
Central Office Bids and Awards Committee (COBAC) Secretariat,
Procurement and Logistics Service, 2nd Floor, Building 25, San
Lazaro Compound, Sta. Cruz, Manila 1003 for evaluation and
subsequent issuance of Certificate of Registration if found in
order:
A.
Requirements common to all suppliers, contractors, consulting
services:
-
Department
of Trade and Industry (DTI) business registration or
Securities and Exchange Commission (SEC) registration
certificate, whichever is appropriate under existing laws of
the Philippines;
-
Mayor's
permit/Municipal license;
-
Taxpayer's
Identification No. (TIN);
-
Sworn
statement of the prospective bidder that it is not
"blacklisted" or barred from bidding by any
government agency, office or corporation, including case of
civil works; non-inclusion in the "blacklist" of
contractors issued by the Construction Industry Authority of
the Philippines
B.
Additional requirements for Drug, and Other Health Commodities
Suppliers:
-
Certificate
of Product Registration (CPR) of product lines (for drugs and
medicines) carried by the supplier;
-
Accreditation
Certificate from DOH as drug supplier or as
medical/hospital/laboratory equipment supplier;
-
Bureau
of Food and Drugs (BFAD) Certification of CGMP compliance.
C.
Additional requirements for consulting services:

Valid
joint venture/consortium agreement in case of joint
venture/consortium;
-
Statement
of consultants of all its completed contracts in the last 3
calendar years or more and all its on-going or committed
contracts with the following minimum information: Name and
location of project, date, type, amount, duration of contracts
and certificate of satisfactory completion if already
completed;

Types
and number of equipment owned, under lease or purchase
agreement;
-
Audited
financial statement for the last 3 years or more, that is BIR
stamped "received";
-
Consultant's
statement on its technical competence, experience, staff
capabilities supported by curriculum vitae, professional
registration certificate, if applicable citizenship, etc.
The prospective supplier/contractor or
its duly authorized representative shall certify under oath that
each of the documents submitted is an original copy or a true and
faithful reproduction or a copy of the original.
Registration shall start on January 27, 2003.
All inquiries relative to the above invitation shall be addressed
to the Chairperson of the COBAC Secretariat.
